衛生福利部執行命令：林民莒違反醫師法廢止中醫師證書，請於本執行命令送達之次日起15日內，前來本部辦理繳回中醫師證書。若因故無法親自前來，得委託他人或掛號郵寄回本部辦理

MINISTRY OF HEALTH AND WELFARE Execution decree is hereby given, for canceling the Doctor of Chinese Medicine license from Lin Min-chu for violating "Physicians Act"; Lin shall return the license to the Ministry within 15 days from the next day following the delivery of the decree; Lin may commission the third party to return the license or send it by registered mail to the Ministry if he is unable to appear in person
